An Authorization for Vehicle Services Form REG0169 is required if doing one of the following services on behalf of someone else, or if the vehicle/plate in question is registered in two names:
- New Plate (First Time Registration)
- Renew Registration
- Plate Transfer or Exchange
- Duplicate Registration
- Tab Replacement
- Cancel Plate/Registration
